It was another guild's applicatin form which has been disbanded years ago :P I just kept it for my self. FYI, when I applied other guilds most of them had their own application form :)

I had every card, my brother had a gf who worked at a toy store where she could take as many pokemon card packs she wanted to because nobody bought them anyway.
